Description: CCHC provides behavioral health and access to social services and has Licensed Clinical Social Workers (LCSWs) available at each site. Services include: individual counseling, child and adolescent counseling, lifestyle coaching, goal management, referrals to treatment and patient navigation services to assist with off-site referrals.
Eligibility: CCHC accepts most insurances and payer sources including: HMO, PPO, Medi-Cal, Medicare, CHDP, Covered California, CPSP, Family PACT, My Health LA, Presumptive Eligibility.
