Eligibility: - All ages; Served in the active military, naval, or air service; Did not receive dishonorable discharge; If veteran enlisted after September 7, 1980, or entered active duty after October 16, 1981, must have served 24 continuous months or the full period for which they were called to active duty unless any of the following are true: Were discharged for a disability caused or made worse by active duty service, discharged for a hardship or "early out" or served before September 7, 1980; If the applicant is a current or former member of the Reserves or National Guard, applicant must have been called to active duty by a federal order and completed the full period for which they were called or ordered to active duty; Cannot have active-duty status for training purposes only
Requirements: Social Security Number for applicant, spouse and qualified dependents; Military discharge papers (DD214 or other separation documents); Insurance card information for all insurance companies that cover applicant including any coverage provided through a spouse or significant other (includes Medicare, private insurance, or insurance from employer); Gross household income from the previous calendar year for applicant, spouse and dependents (includes income from a job and any other sources, Gross household income is income before taxes and any other deductions); Applicant's deductible expenses for the past year (includes certain health care and education costs)
