- Description: Provides one-on-one parenting program, by phone and/or virtual. The program implements Parents as Teachers (PAT) model, which is designed to support parenting and the child's development. Visit also includes parent and child interactive activity and case management support. Parents can be enrolled in the program for 2 years, and visits occur twice per month.
