Description: Offers advocacy, assessment, facilitation, and resource referral to older adults. Through case management, home visits and telephone contact, resources are provided to seniors to help them meet their health and wellness needs. Services may include: Home safety assessments Assessment of needs Home visits and telephone contact Community referrals Friendly visitor services Assistive devices for daily living Utility discount services Food/nutrition programs (i.e. Home Delivered Meals)
Eligibility: - 60 years and older, Must be Brea resident, Financial income may be required when determining individual's eligibility for some community programs, Non-Brea residents of any age welcome to call or come in for resource information for themselves or older adult loved ones
Requirements: Intake and Enrollment Forms are completed with the case manager during the first scheduled appointment
