Description: Provides assistance to people who are being released under AB109 (Parole, PCS, Mandatory Supervision (MS) with: Help obtaining Right to Work documents (Birth Certificate, California ID, Social Security) Assistance with Child Support (obtaining incarceration dates from both local jail and state prison to reduce amount owed to child support under the Compromise Of Arrears Program) Assistance with identity theft (obtaining local jail and state prison records to report identity theft while incarcerated to the 3 credit bureaus) Filling out Prop 47 and Prop 64 to check if there are any qualifying cases for relief (eg. Marijuana related cases, drug possession, theft under $950, etc) Completing SSI applications and obtaining medical records. Obtaining copies of Immigration Documents (eg. Permanent Residence replacement card, Citizenship certificate replacement, etc) Completing SSI applications and obtaining medical records. Obtaining copies of Immigration Documents (eg Permanent Residence replacement card, Citizenship certificate replacement, etc) Dental and Vision resources covered under MediCal. Assistance with animal companions. Referrals to the Department of Rehabilitation for paid vocational training and resume assistance. Assistance with glasses and dentures Education referrals
