• Requirements: Proof of Native American heritage, Proof of residence within coverage area listed below, Government-issued picture ID, Proof of household income for the previous six-month period, Proof that current household income is enough to maintain suitable living conditions, Proof of being 18 years and older or being an emancipated youth, Detailed explanation of the unplanned and/or unintentional circumstance(s) that resulted in an emergency situation
