• Description: Provides Section 8 Housing Choice Vouchers (HCV) for non-elderly disabled families. The vouchers assist people who are non-elderly disabled (NED) and living in nursing homes (or other long-term healthcare facilities), to transition into the community with appropriate services. • Eligibility: - 18-61 years, Must be head-of-household or spouse; Must have disability; Must require home and community based services to live in the community; Must have continuously lived in an in-patient facility in Orange County for at least 90 days (e.g. Hospital, Nursing Facility, Intermediate Care Facility for Developmental Disability, or Institution for Mental Condition); Excluding rehab days, must have completed at least 90 days after rehabilitation services reimbursed by Medicare; Or must have 90 days from the date of admission and no rehabilitation services have been received; Must have been receiving Medi-Cal for at least one day; Must receive Medicaid reimbursement for inpatient services
