Description: Provides long-term case management services to CalOptima health clients. Services include:Support to secure and maintain housing A variety of other community-based services to help address members' health-related social needsConnections to quality care Referrals to other CalAIM services including medical case management and day habilitation Referrals to potential permanent housing opportunities (if eligible and opportunities are available): assistance in searching for a rental unit deposit and furniture assistance (for new permanent housing) Case management for 6 months to 1 year after securing housingNote: No immediate emergency rental assistance or financial assistance is available.
Eligibility: - Adults: 18 years and older, Families with minor children that are 18 years and younger; Must have Orange County CalOptima insurance; Must be experiencing homelessness or at risk of homelessness (at-risk requires eviction notice or letter from landlord)
