Aims to keep the members' current home environment and works to remove any barriers that might jeopardize their staying within their homes. Designed to guide members in addressing the unique needs of Medi-Cal Managed Care Plan members and the factors that strongly influence people's health and overall wellness, including working on needs such as housing, food, and financial stability. The program is supported by CalAIM, also known as the Medi-Cal Transformation Initiatives.
