Description: Places adults and families experiencing, or at risk of, homelessness, with a psychiatric disability in appropriate supportive housing when available.Owns and provides property management to housing facilities. The number of units or bedrooms available at any given time is limited. Hours: Coordinated Access System: Seven days a week, 24 hours;Hope Cooperative Office: Monday through Friday, 8 am - 5 pm
Eligibility: Must be 18 years of age or older, homeless, and diagnosed with a psychiatric disability and receiving services from Sacramento County Behavioral Health Services. Income limits apply and are aimed at individuals and families earning below 30% of Annual Median Income (AMI).
Requirements: Income verification and documentation that the individual is receiving services from Sacramento County Behavioral Health Services.
