Description: Home-based services provided to pregnant women and children ages birth to 36 months old and their families. A home educator makes weekly visits to provide age and developmentally-appropriate child-focused learning experiences in the family's home.Services include: Activities to promote child growth and development Individualized education plan Health, nutrition, and developmental screenings Parenting skills support Services and support for children with disabilities Referrals and access to resource staff Prenatal and postpartum information for pregnant moms Preschool readiness activitiesSocialization gatherings, conducted twice a month, allow children and families to socially interact and learn a variety of skills about topics important to the educational development and education of the child.
Eligibility: Low-income pregnant women and families with children up to three years of age. Focuses on children with an IFSP (Individual Family Service Plan). This is a plan for special services for young children with developmental delays or disabilities.
Requirements: Must fill out the SCOE Early Head Start Home Based Referral/Interest Form.
