  • Eligibility: Students enrolled at ARC who have been diagnosed with a disability.
  • Requirements: Application for DSPS Services. Copies of records verifying the student's disability. Documentation should specify the disability diagnosis and ideally, any disability-related educational or functional limitations. Common forms of disability verification include the following:. A note from a healthcare provider stating the disability. Medical records containing the diagnosis of a disability. A note from a psychologist or counselor identifying the disability diagnosis. Records from high school documenting a disability, such as an Individualized Educational Plan (IEP), 504 Plan, or Psychoeducational Evaluation. Previous learning disability or cognitive testing results. Other similar documentation to be reviewed on a case-by-case basis
