Description: A 12-month transitional housing program for homeless families receiving CalWORKs. Provides a safe environment for families to live in while they seek permanent housing and employment. Services include individual and group counseling, life skills and parenting classes, and referrals for permanent housing.One-, two-, and three-bedroom apartments are available based on the size of the family. Each family has their own unit.
Hours: Administration: Monday through Friday, 8 am - 5 pm
Eligibility: Families (single mothers with children, single fathers with children, and two-parent households) who meet all of the following requirements:- One adult member of the household must have CalWORKs for themselves and their child(ren). The adult cannot be timed out and would ideally have at least 12 months of CalWORKs remaining.- One adult member of the household must have had an alcohol or drug issue. The adult must have completed a Sacramento County treatment program within 12 months of intake and/or is currently engaged in drug treatment services. Adults must maintain their eligibility throughout the length of the program. Since the DHS Program is a sober living environment, regular drug testing is conducted.
Requirements: - Completed referral form- ID and social security card- Current TB test result- SUD Assessment, ASAM, or letter from a treatment facility on their letterhead stating dates of treatment and graduation date (if applicable)- CalWORKs benefit verification and CalWORKs time clock showing how many months of CalWORKs have been used- Birth certificates and immunization records for children (if applicable)
