Hours: Monday through Friday, 8 am - 5 pm;Closed on state holidays
Eligibility: - Must have a physical or mental impairment that substantially impedes his or her ability to secure employment- Require vocational rehabilitation services to prepare for, secure, retain, or regain employment consistent with the applicant's unique strengths, resources, priorities, concerns, abilities, capabilities, interests, and informed choice- Program participants are expected to be available, responsible, active and dedicated contributors to their own success
Requirements: For intake appointment, bring:- Any documents you have about your disability- Recent records and benefit letters (i.e.; Social Security Administration, Ticket to Work Program, or another referral)- A list of names, addresses, and telephone numbers of doctors, professionals, and organizations you have consulted about your disability
