Description: Provides case management services and support to families and at-risk youth suffering from mental health conditions. Helps youth learn new skills to decrease their mental health symptoms and risky behaviors while building on their strengths to improve their overall well-being and behavioral health.Therapeutic services may be provided in the family's home, school setting, community setting of the family choice, or counseling center.Case Management Services include: Psychiatric evaluations and medication management. Attendance and advocacy at Individualized Education Plan (IEP) meetings with schools. Attendance and advocacy at Student Study Team (SST) meetings with schools. Attendance and advocacy at Team Decision Making (TDM) meetings with Child Protective Services (CPS). Linkage to community-based resources for transportation, food, clothing, and shelter. Collaboration with schools, doctors, hospitals, social services, child welfare, courts, Alta Regional Center, Alcohol and Drug Services (ADS), and Juvenile Justice.Must have a referral from Sacramento County's Access Team.
Hours: Sacramento County's Access Line: Monday through Friday, 8 am - 5 pm;Stanford Sierra Youth & Families: Monday through Thursday, 8:30 am - 5:30 pm; Fridays, 8:30 am - 5 pm
Eligibility: Medi-Cal eligible children and youth ages 0 - 18 (and in some circumstances up to age 21). Must have a referral from Sacramento County's Access Team.
