Medication ManagementPrescription and prescribed over-the-county medications (no co-pay) In-Home AssistanceMedical care and personal care at home, including medical equipment and supplies. Social EngagementRecreational activities, meals, and entertainment. Transportation:Unlimited approved transportation to and from the center or medical appointments. Services include but are not limited to those listed above.
Eligibility: To qualify for PACE, seniors must meet certain financial and medical requirements.. At least 55 years old. Eligible for nursing facility level of care. Able to live safely at home and in the community with support. Living in an InnovAge PACE service area
Requirements: Social Security Number, Proof of income/resources, Proof of date of birth, and Health Insurance card (Medicaid/Medicare) information.
