  • Eligibility: Individuals, couples, children, and adolescents living with HIV/AIDS. Eligibility for enrollment will be verified at the initial intake appointment.Individual must:- Be HIV positive.- Reside in Sacramento County (residents of El Dorado, Placer and Yolo counties may be eligible if no other HIV mental health services are available).- Have a gross income that does not exceed 500% of the Federal Poverty Guidelines.- Have health insurance (for those who do not have insurance, Sunburst Projects case managers can help them obtain health insurance).
  • Requirements: Provide at least one document for each of the following:1. Proof of HIV-positive status such as HIV positive lab results, letter from one's physician or licensed health care provider, or CDPH Form 8440.2. Proof of residence such as a driver's license, ID card, utility bill, rental or lease agreement, etc.3. Documentation of income such as a pay stub, W-2, etc.4. Proof of insurance status such as an insurance card, denial letter from Medi-Cal, etc.Call, email, or visit the website for more information.
