Services and enrollment are offered year-round.Health Navigators can help with: Enrollment for health, dental, and nutrition coverage Finding a Medi-Cal doctor, Eye doctor or dentist Scheduling health appointments Arranging transportation to health appointments Arranging language interpretation Connecting to other health-related services in the areaMedi-Cal members can request transportation services through their health plan. Transportation is available on a case-by-case basis. Call 916-414-8333 at least five days in advance to schedule a ride to see a doctor.
Hours: Office Hours: Monday through Friday, 9 am - 5 pm
Eligibility: Open to all.
Requirements: - Photo ID- Proof of address (utility bill, rental agreement, etc.)- Proof of citizenship or immigration status (naturalization certificate, green card, employment authorization, etc.)- Proof of income (federal tax information from recently filed tax return; bring all pages)- Social security number and date of birth for all tax dependents/household members (even if they are not applying for health insurance)
