Description: Offers a full range of quality home health care options for patients recently discharged from the hospital, as well as homebound individuals who need additional home health care. Hours: Administration: Monday through Friday, 8:30 am - 5 pm
Eligibility: Dignity Health patients who have recently been discharged from the hospital, and need short-term skilled nursing, physical therapy, or speech therapy. Individuals are eligible for home health services if they:- Are an ill or frail elderly individual, or- Are a mother of a child under 18, or- Are an infant, or- Are a child with special needs
