Description: Strives to find permanent families for the children in the program. Focuses on strength building to improve the well-being of the entire family. The process "wraps" each child and family with tailored services to foster solutions and provide ongoing case management.Services and goals include: Create a strength-based plan while promoting the family voice and choice.Develop, implement, and regulate the individual plans for each family. Provide services and support to families so that their children can return to live in a safe, stable, and permanent family home. Move children from group care to family-driven care and shorten the amount of time each child is in the Residential Treatment Program.
Hours: Office Hours: Monday through Friday, 8:30 am - 5 pm
Eligibility: Children ages 0 - 24 years who are referred through Sacramento County by Child Protective Services, Probation, AAP (Adoption Assistance Program), and/or Mental Health Providers.
