• Eligibility: Must meet the following requirements:- Must be living in a community served by Lions Eye Foundation for a continuous year- Must not have any health insurance (no county, government, or private coverage; no partial coverage, share-of-cost, or deductibles)- Adjusted Gross Income (AGI1) must be within the guidelines listed below (effective as of January 1, 2024).Income-Based Eligibility Guidelines:Household size of 1: monthly income of $4,860; annual income of $58,320Household size of 2: monthly income of $6,573; annual income of $78,880 (with two incomes)Household size of 3: monthly income of $8,287; annual income of $99,440Household size of 4+: monthly income of $10,000; annual income of $120,000
