• Eligibility: To be eligible, an individual must meet the following eligibility requirements: - Be a Medi-Cal member with Anthem Blue Cross- Reside in a nursing home within Sacramento County for 60 or more days- Currently receive medically necessary nursing services from that facility- Be willing and able to safely move homes
