Eligibility: To qualify, you must be:- Eligible or receiving Medi-Cal - Enrolled in participating Managed-Care-Plan (Anthem or HealthNet)- Receiving some income- Either a patient in a Skilled Nursing Facility, or at-risk for Long-Term Care
