Description: Assists members who are living at home or in a nursing facility in transferring to an assisted living facility to live in their community and avoid institutionalization in a nursing facility, when possible. Assisted living facilities provide services to establish a community facility residence such as support with daily living activities, medication oversight, and 24-hour onsite direct care staff.How Nursing Facility Transition to Assisted Living Works: Assesses the client's needs, and personal preferences (location and other specifics).Helps the client apply for income programs.Matches the client's needs and preferences with a variety of local Assisted Living communities.Agreement - the client agrees to move to the Assisted Living Community / Master Care coordinates agreements.Helps in the safe transition to the client's new Assisted Living home.The program is supported by CalAIM, also known as the Medi-Cal Transformation Initiatives. Eligibility: Nursing Facility Transition to Assisted Living:- A Medi-Cal member with HealthNet or Anthem Blue Cross- Some income - Can be Social Security, General Assistance, or other sources- Willingness & ability to pay 90% of all income to Assisted Living as your "Room & Board"
