Description: Assists members in transitioning from a nursing facility to a private residence by providing funding for set-up services such as security deposits, set-up fees for utilities, and health-related appliances, such as air conditioners, heaters, or hospital beds.How Nursing Facility Diversion to Home Works:Assesses the client's needs, their home, and other specifics.Helps the client access Home Modifications (like ramps and grab bars), personal caregiving services, medically-tailored meals, and more. Provides up to $7,500 in home modifications.Assures the client's needs continue to be met through transition and beyond.Coordinates together with the client's care team to assist in the safe transition to home.The program is supported by CalAIM, also known as the Medi-Cal Transformation Initiatives.
