Description: Provides individualized care services for seniors to live independently in the community. Program coordinates care services based on the individual's medical, functional and social needs. PACE is both a care provider and health insurer.
Hours: Intake Line: Seven days a week, 24-hours;Elk Grove Center: Monday through Friday, 8 am - 4:30 pm
Eligibility: Individuals must meet the following criteria:- Reside within Sacramento County - Be 55 years of age or older- Receive Medi-Cal or Medicare- Certified by the state as eligible for nursing facility level of care (In California, a doctor determines if there's a need for nursing home care. People may need nursing home care if they can't self-administer medications or feed themselves, or if they need help dressing/bathing or suffer from a mental illness or Alzheimer's.)- Live safely in their home and the community
Requirements: Photo ID and proof of Medi-Cal and/or Medicare coverage.
