• Description: Offers Medi-Cal members experiencing homelessness or at risk of experiencing homelessness assistance in finding, applying for, and securing housing. Assistance includes assistance in searching and applying for housing, housing tenancy support, housing sustaining services, and connection to resources. Assistance does NOT include first or last month's rent.Helps individuals and families follow a path to attainable and sustainable housing. Individuals must participate in services that come with any funds awarded. The program is supported by CalAIM, also known as the Medi-Cal Transformation Initiatives.
