• Description: Community Health Worker (CHW) services help clients meet their health goals and coordinate care.CHWs can help with a range of needs, including:Providing vetted information or instruction on health topicsCoaching and goal setting to improve an individual's health or ability to self-manage health conditionsAccessing and understanding healthcareConnecting to community resources and services, including medical translation/interpretation, transportation services, or health-related social needsHaving a cultural liaisonPerforming outreach and resource coordination around preventive servicesHelping with enrollment in government or other assistance programsProviding peer support.Community Health Worker services do not include:Clinical case management or care management that requires a licenseChild careChore services, including shopping and cooking mealsCompanion servicesEmployment servicesDelivery of medication, medical equipment, or medical supplies.The program is supported by CalAIM, also known as the Medi-Cal Transformation Initiatives. • Eligibility: To be eligible, an individual/family must meet the following eligibility requirements: - Be a Medi-Cal member with Molina Healthcare, Kaiser Permanente, HealthNet, Anthem BlueCross, or Partnership HealthPlan of California.- Live in the state of California.- Have one more chronic condition (including mental health) OR exposure to violence and traumaAND- Have challenges in meeting their health or additional needs.Individuals cannot receive CHW services if they are already enrolled in Enhanced Care Management (ECM).
