Aims to keep the members' current home environment and works to remove any barriers that might jeopardize their staying within their homes. Designed to guide members in addressing the unique needs of Medi-Cal Managed Care Plan members and the factors that strongly influence people's health and overall wellness, including working on needs such as housing, food, and financial stability.   • Eligibility: To be eligible, an individual/family must meet the following eligibility requirements: - Be a Medi-Cal member with Anthem Blue Cross, HealthNet, Kaiser Permanente, or Partnership Health.- Live in a County within the service area.- Be at risk of experiencing homelessness.
