Provides support with enrollment for health, dental, and nutrition coverage; helps find Medi-Cal doctors, eye doctors, and dentists; helps schedule appointments; provides language interpretation; and connects individuals to local health-related services and community resources. Offers transportation arrangements on a case-by-case basis.Assistance is free and available year-round.

• Requirements: One document type for each of the following:- Proof of Identity: passport, driver's license, ID card, or other photo ID.- Proof of Citizenship or Immigration Status: passport, permanent resident card, or certificate of citizenship/naturalization. - Proof of Income: pay stubs or income tax return from previous year.- Proof of Social Security Number: social security card or tax form(s).- Proof of American Indian Alaska Native Status (if applicable): tribal enrollment/membership card.- Medi-Cal card (if available).- Proof of address.Documents required may vary depending on the service the individual is applying for.
