• Description: An annual Christmas Basket program for individuals, families, and seniors living within zip codes 95815, 95833, 95834, 95835, and 95838, and residents of Mirasol Village.General Information: Only those who can prove that they live in the household may be included No changes/additions can be made after the application has been completed Only one basket per household/address will be provided. Families who are sharing housing will receive one basket based on the number of people living in the house At the time of registration, families will be given an appointment day and time in December to pick up their baskets.If a household is receiving Christmas assistance from another agency, they will not be eligible to receive a basket.

• Eligibility: Residents of zip codes 95815, 95833, 95834, 95835, and 95838, and residents of Mirasol Village.If a household is receiving Christmas assistance from another agency, they will not be eligible to receive a basket.
• Requirements: . Photo ID (for the adult applying). Proof of address for each adult (18+) in the household: Bring two CURRENT (September/October/November/December 2025) items with name & address, such as utility bills, phone bills, letters with legible postmarks, etc.. For each child ages 5-17 years in the household: Document showing name, date of birth, and address (2025 school registration, doctor's office printout, Medi-Cal renewal letter) . For each child 4 years & under in the household: Birth certificate.Please note: school IDs, vaccination records, Social Security cards, and Medi-Cal cards do not have addresses and cannot be used for proof of address for children ages 5-17.
