  • Requirements: Must bring all of the following:1. Birth certificate, green card, or report card for each child2. Current Cal-WORKS aid verification summary card3. Current photo ID/driver's license from the head of the household and the person applyingMust bring ONE of each of the following:4. Proof of ID: picture ID, driver's license, or state ID verification5. Proof of Residence: rent receipt or current utility bill6. Proof of Income: paycheck stub/EBT, SSI verification, or current student aid
