Description: Provides services to individuals who are providing caregiving services to older adults.
Services include:
• Assistance to caregivers through contracted providers
• Comprehensive assessment
• Care management
• Professionally administered support groups and counseling
• Caregiver skills training
• Respite care
• Legal services
• Minor Home Modification (grab bars, safety checks, etc.)
• Grandparent caregiver support
• Outreach and education
Eligibility: Adult family member or individual (18 years and older) who is providing care-giving services to an older adult (60 years and older).
Grandparent (60 years and older) or an older adult who lives with a child (younger then 18 years) and is designated as the primary caregiver for the child.
Priority is given to individuals with the greatest social and economic needs.
