Description: An integrated care peer and family support and engagement program based at San Diego County Psychiatric Hospital. The staff empowers individuals by modeling and assisting in the development of self management skills. The program links individuals to outpatient and community services and support.
Program services include information on community resources, participant-directed case management, family education, and peer support.
Eligibility: Must be an adult 18 years and older with mental health and/or alcohol and drug issues being served at the San Diego County Psychiatric Hospital or referred from East County Mental Health Center, North County Mental Health Center or Southeast Mental Health Center.
Will also serve family members or loved ones of those in Next Steps.
