• Description: Provides services and information to patients and their families to help understand the physical, emotional and social challenges that occur with serious illness. Services include: Assistance and guidance to improve quality of life Open discussion about treament choices, including treatment for client's disease and management of symptoms Coordinate care with all clients health care providers Emotional, physical, psychological and spiritual support Nursing - in-home visits, 24-Hour phone support, medication education and education on disease and disease process. Social Services - assistance with community resources, in-home visits and emotional support Chaplain Services - spiritual support and in-home visitsCare is provided at the patient's home, a skilled nursing facility, or residential care home (assisted living facility).No hands-on medical care is provided.
