  • Eligibility: Must have a life expectancy of six months or less (based on their physician's prognosis) orAnswer yes to four of following questions:*have disease or injury that cannot be cured*spend most of the day in bed or in a chair*have to take pills for very bad physical, body pain*have made a lot of trips to the emergency room in the past 6 months*doctor informed that my life expectancy is limited*need help from other people with daily activities like taking a shower, getting dressed, eating, cooking, walking, or getting out of bed*started feeling more tired and weak*lost a lot of weight in the last six months
