• Eligibility: Must be under 21 years of age who have a CCS eligible medical condition and reside in San Joaquin County.Qualifying conditions such as:. congenital heart disease. cancers, tumors. hemophilia, sickle cell anemia. thyroid problems, diabetes. serious chronic kidney problems. liver or intestine diseases. cleft lip/palate, spina bifida. hearing loss, cataracts. cerebral palsy, uncontrolled seizures. rheumatoid arthritis, muscular dystrophy. AIDS. severe head, brain, or spinal cord injuries, severe burns. problems caused by premature birth. severely crooked teeth. broken bonesThere is a financial eligibility requirement for specialized equipment.*Family annual income must be under $40,000
• Requirements: Medi-Cal card and current medical documentation of medically eligible condition. Non Medi-Cal applicants: Social Security Card, Driver's License, rent and/or utility receipts/bills, check stubs, health insurance information, and State and Federal Income Taxes.
