Head injuriesThere is a financial eligibility requirement for specialized equipment.*Family annual income must be under $40,000
• Requirements: Medi-Cal card and current medical documentation of medically eligible condition. Non Medi-Cal applicants: Social Security Card, Driver's License, rent and/or utility receipts/bills, check stubs, health insurance information, and State and Federal Income Taxes.
