Description: Provides a variety of services to help caregivers care for their loved ones.Services include: Family Consultation and Case Management Services - Provides assessment of caregiver needs, short or long-term care planning, and ongoing consultation. Provides short term counseling to caregivers seeking emotional support and skill development to better assist caring for their loved ones. Education and Training - Provides information on chronic and disabling condition and disease. Provides training on related caregiving issues such as stress reduction and self-care. Legal and Financial Consultation - Provides an attorney to address legal and financial issues, could include planning for public assistance programs, understanding Advance Healthcare Directives and Powers of Attorney. Respite Care - Provides care to loved one to give caregiver opportunity for relief and rest. Service can be provided in the individual's home or in an out-of-home setting. Short-term Counseling - Offers up to 6 one-hour individual counseling sessions to caregivers looking for emotional support, skill development and strategies to better cope with their caregiving situation.
Eligibility: Must meet one of the criteria:*Caring for someone at least 18 years of age with a cognitive impairing condition (dementia, Alzheimer's disease, stroke, traumatic brain injury)
