• Eligibility: Must meet eligibility requirements:-Must be a Medi-Cal member with Kaiser Permanente-Must live in the state of California-Must have one more chronic condition (including mental health) OR exposure to violence and trauma AND have challenges in meeting their health or additional needs. *Individuals cannot receive CHW services if they are already enrolled in Enhanced Care Management (ECM).
