• Eligibility: Must meet eligibility requirements:-Must be a Medi-Cal member with Health Plan of San Joaquin, HealthNet, or Kaiser Permanente-Must live in the state of CaliforniaAnd meet one of the following requirements:-Experiencing homelessness or at risk of becoming homeless-Someone who frequently needs to visit the emergency room or hospital because of an ongoing health-Living with serious mental health or substance use disorder needs-A current or former child or youth involved in the child welfare system-A pregnant or postpartum individuals (up to one year after giving birth) who is Black, American Indian, Alaska Native, or Pacific Islander-An adult with complex needs who need a high level of support in their life ranging from health to social services
