  • Description: Provides a wide array of services for individuals with developmental disabilities. Includes diagnosis and assessment of eligibility as well as planning, access, and care coordination. Each individual is assigned to a specific service coordinator. Service coordinators draft a one-to-three-year plan of the individual's needs and requested services. Plans are altered as needed to continue to fit an individual's needs.
