Description: Provides a wide array of services for individuals with developmental disabilities. Includes diagnosis and assessment of eligibility as well as planning, access, and care coordination. Each individual is assigned to a specific service coordinator. Service coordinators draft a one-to-three-year plan of the individual's needs and requested services. Plans are altered as needed to continue to fit an individual's needs.
Eligibility: Individuals of any age must have or be at risk for a developmental disability as defined by California law through the Lanterman Developmental Services Act.
Disability must have originated before the age of 18 years old.
Must be likely to continue indefinitely, and constitute a substantial handicap in 3 or more major life areas.
