  Provides a low-cost health coverage program to adults in Santa Clara County. Helps individuals without health insurance who are ineligible for Medi-Cal or Covered California subsidies. Enrollees choose a community clinic to receive primary and preventive care services. Offers medications are available through Valley Medical Center's pharmacy.
