• Requirements: Opening a child support case: - Child(ren)'s birth certificate(s) - Your child support order, if you have one - Your marriage license or certificate, if you have one - Information regarding health insurance for the children - Social Security number of the NCP (non-custodial parent) - Current physical description or photograph of the NCP - Information pertaining to the NCP's income and other assets - Divorce decree or separation agreement, if you have one - Name and address of current or most recent employer of the NCP - Full legal name, date of birth, address, and phone number of the NCP - Names of friends and relatives and agencies to which the NCP might belong
