Must be Kern County resident. (only for medical diagnostic exam financial assistance)

Requirements: - Copy of identification
- Most recent rent receipts
- Utility receipt (showing residency address)
- Employment verification for all who work in the household
- Most recent tax returns for all the taxpayers of the household
- Must have a medical diagnosis from a family doctor or specialist
- Last 3 months of check stubs for everyone who works in the household
- Checking and Saving bank account statements for the last 3 months (if possible)
