Program administers the Medi-Cal program in 14 counties across Northern California. Ensures that all members have access to health care and can choose a primary care provider (PCP). Website offers directory of primary care providers (PCP's). Also offers application assistance for Medi-Cal.
Services include:
Free 24/7 Advice Nurse line - Patients may use this service for medical questions, or if unsure about whether to go to the emergency room. (If one has a life-threatening emergency, one should go to the closest emergency room or call 9-1-1.)
Non-medical transportation (NMT) - Allows one to use a car, taxi, bus, or other transportation. Members can use NMT when traveling to and from a medical appointment. Provides mileage reimbursement to friends or family members. Call Care Coordination at least five business days before appointment. For urgent appointments, call as soon as possible.
Non-emergency medical transportation (NEMT) - for disabled persons, unable to use car, bus, train, or taxi. Must request service through one's doctor. NEMT is an ambulance, litter van, wheelchair van, or air transport. Call Care Coordination at least five business days before appointment. For urgent appointments, call as soon as possible.
