Provides short-term care management for frail seniors. Helps them live at home rather than be placed in a facility. Case managers connect clients to local programs that meet the clients' needs. Offers money management services for seniors at risk of losing their housing.

Eligibility: Sonoma County residents 60 years of age or older. Require help due to an illness, injury, disability or major life event, in order to live independently. Clients must NOT be receiving care through the Department of Rehabilitation, Department of Mental Health, Department of Developmental Disabilities, or Multipurpose Senior Service Program (MSSP).
