HOME & COMMUNITY-BASED ALTERNATIVES WAIVER, COUNTY OF SONOMA HUMAN SERVICES - ADULT AND AGING DIVISION

Description: Offers an alternative to a nursing home, or institutional living. Helps put supportive services in place to allow medically fragile individuals to stay at home. Supports residents wishing to return home from a nursing home or institutional facility. Works together with a social worker and a nurse to determine the services the client needs. Offers regular visits to clients to provide ongoing support, information and referrals.
