- Description:
Provides at home support for pregnant and/or parenting women with children under 5 years of age. Offers home visitation services from a public health nurse. Meets with families one to two times per month. Nurses and community health workers are trained to help families who have experienced trauma. Assists with the following:
- Education about pregnancy, birth, breastfeeding, newborn care, and parenting
- Support for complex medical or social situations
- Mental health screening and referral
- Child development screenings and referrals to community services
- Referrals to community resources including housing, food, and childcare
- Legal assistance
