Provides medically tailored meals and groceries to individuals with serious medical conditions. Offers home delivered groceries to clients who are homebound or homeless. Also offers nutrition counseling. Includes the following services:
HIV Nutrition - Provides medically tailored weekly groceries for individuals with a positive HIV diagnosis. Offers service for as long as needed.
Welcome Home - Provides three months of medically tailored groceries and/or meals following an in-patient hospitalization.
Infectious Disease - Provides medically tailored groceries delivered weekly. The duration is determined on a case-by-case basis.
Oncology Nutrition - Provides three months of medically tailored groceries or meals for the client and their household. Service duration may be extended based on the course of treatment.
Full Belly - Provides medically tailored groceries and meals for expecting clients and their household. Service may continue until the baby reaches 3 months of age.
Community Supports - Provides twelve weeks of medically tailored groceries and meals for patients with Medi-Cal. Clients must have medical justification for the services.
Bags of Love - Provides ready-to-eat meals for organizations to distribute to their unhoused patients.
Eligibility: Residents of Sonoma County living with HIV/AIDS and other serious illnesses. Illnesses/ circumstances include:
-HIV/AIDS
-Recent hospitalization within the past two weeks
-Individuals with Medi-Cal who have diabetes
-COPD
-Cancer
-Kidney
-Congestive heart failure
-Severe malnutrition
